Fall prevention for older adults, in the home


Effective fall prevention treats the causes, not just the trip hazards. Tandem assesses leg strength, balance recovery, gait, blood pressure response on standing, medication effects and the home environment itself, then trains the strength and balance that let a person catch themselves. Removing rugs alone does not do that.

Why hazard removal is only half the job

Clearing rugs and adding grab bars reduces the number of times someone is destabilised. It does nothing about what happens when they are destabilised anyway, which they will be.

The ability to recover from being off balance is a physical capacity. It depends on leg and hip strength and on reaction, and it is trainable. That is the half of fall prevention most programs skip.

Fear of falling makes falling more likely

This is counter-intuitive and it is well established. Fear reduces activity. Reduced activity costs leg strength and balance. Lost leg strength and balance cause falls. Being more careful accelerates the very outcome it is trying to avoid.

The intervention is supervised challenge: a therapist present while the person does slightly more than feels comfortable, repeatedly, until the comfortable range expands again.

What an in-home assessment finds that a clinic cannot

The height of the chair they actually sit in. Whether the light switch is at the wrong end of the hallway to the bathroom. Which stair the rail stops short of. The threshold they catch a toe on. The bath they climb into holding a towel rail that is not rated to hold them.

A clinic can measure a person. Only the home reveals the specific conditions that will produce the fall.

  • Leg and hip strength, including repeated sit-to-stand without arms
  • Balance, including recovery from being deliberately destabilised
  • Gait: step length, foot clearance, shuffling, need for walls or furniture
  • Blood pressure response to standing up
  • Medication effects that cause dizziness or drowsiness
  • Stairs, lighting, thresholds, rugs, chair and bed height, footwear, bathroom transfers

Questions about falls and balance


What is a fall risk assessment?

It is a structured evaluation of the things that make a fall likely: leg strength, balance, gait, vision, blood pressure response to standing, footwear, medication effects and the home environment itself. Done in the home, it produces a specific list of what to change rather than a general warning to be careful.

What are the warning signs before a first fall?

Reaching for furniture while walking, taking longer to stand up from a chair, shuffling or catching a toe, avoiding stairs, holding the wall in a hallway, and giving up activities that used to be routine. Fear of falling is itself a warning sign, because it reduces activity and accelerates the strength loss that causes falls.

Does being afraid of falling make a fall more likely?

Yes. Fear of falling increases fall risk, because it leads people to move less, and moving less costs leg strength and balance, which are the two things that prevent falls. Breaking that cycle with supervised, progressive strength work is more effective than being more careful.

Can low blood pressure cause falls?

It can. A drop in blood pressure on standing up, called orthostatic hypotension, causes light-headedness in the first seconds after rising, which is exactly when a person is least stable. It is worth raising with the prescribing physician, and it is one of the things a home assessment looks for.
