Medicare physical therapy can end before your loved one is back to their previous level of strength and independence. They may still have weak legs, balance problems, difficulty walking, or need help with everyday activities. Therapy ending doesn't always mean your loved one is where they need or want to be.

Medicare-covered physical therapy is an important service, but it has requirements for when skilled therapy can continue and when coverage must stop. That can happen even when your loved one is still having difficulty with everyday things, like getting up from a chair without pushing with their arms, getting safely up and down the stairs, getting in and out of the car, or walking without relying as heavily on a walker.
Your loved one may have made progress but still need more time to build the leg strength that makes everyday activities easier. Meaningful strength takes time to build, and the work doesn't necessarily stop when therapy does.

We meet your loved one where they are today, and the same physical therapist gets to know what motivates them and what they are capable of doing next. As they get stronger, we adjust the exercises, level of challenge, and pace. When maintaining strength is the goal, we continue working to help them hold onto those gains.
Tandem is private pay, so the program isn't limited by insurance coverage. Our one-on-one, in-home approach allows the work to continue as long as it is helping them build and maintain strength, balance, and function.

Questions about Medicare therapy and what comes after
How much physical therapy does Medicare actually cover?
There isn't a set number of physical therapy visits Medicare covers. Coverage is based on medical necessity and the need for skilled therapy.
A person may still need help getting stronger or moving safely, but Medicare-covered therapy can end when skilled therapy is no longer considered necessary, even when the need for strength and support hasn't gone away.
That gap is one of the reasons Tandem Strength & Balance® was established: to give older adults a way to continue building and maintaining the leg strength they need for everyday life, even after Medicare-covered therapy ends.
Why did my parent get discharged from therapy while they were still weak?
Leg strength is often the overlooked key to functional progress, and building it takes time. Getting up from a chair, climbing stairs, getting in and out of the car, walking safely, and maintaining balance all require enough strength to do them. Not everyone builds strength at the same pace, and some older adults need more time and consistent strengthening to see meaningful changes in function.
Medicare-covered therapy is based on medical necessity and the need for skilled therapy. So therapy can end when those requirements are no longer met, even if your parent is still weak and hasn't made the functional progress your family hoped to see.
Therapy may have ended. Their need to get stronger may not have.
Can we use Medicare therapy and Tandem at the same time?
Most families use Medicare-covered therapy and Tandem at different points in their parent's journey. Medicare-covered therapy often comes after a hospitalization, surgery, or change in function. Tandem provides specialized, ongoing strength and balance training with a physical therapist, helping your parent build and maintain the leg strength they need for everyday life.
There are situations where someone may use both, but that is not typical for our long-term clients. The right approach depends on what your parent needs and the services they are receiving.

Is private-pay physical therapy worth the investment?
That depends on what matters most to your family and how your parent wants to live.
As strength and function decline, your parent may need more help, and more help can mean higher costs at home or a move to a setting with a higher level of care. On the other hand, maintaining leg strength can help your parent maintain function, continue doing more for themselves, and potentially avoid some of those added costs.
For many families, the investment is about more than getting stronger. It's about building and maintaining the strength that helps your parent preserve function, remain independent, and continue living the way they want to live.
